Not one error but a series of miscommunications and mismanagement from all parties at all levels caused the Deepwater Horizon disaster, aggravated by the world’s relentless demand for energy, according to a new book released by Tad Patzek, chair of the Department of Petroleum and Geosystems Engineering at The University of Texas at Austin, and co-author Joseph A.

Tainter, a professor in the Department of Environment and Society at Utah State University.
